Interventions Phase II: 2016/17

The SIMTEGR8 methodology has been used to support the development of the following four patient-centric integrated services being piloted through the Leicestershire BCF:

  • Lightbulb housing support service
  • The intensive community support (ICS beds) scheme
  • Help to Live at Home Service
  • Ambulatory pathway at CDU (Glenfield) hospital admissions avoidance scheme for cardio/respiratory patients
